I have read and understand the CHC Primer including the Statement of Faith, Code of Conduct, Discipline Policy and CHC Payments.

I understand that failure of the student and/or parent/guardian to comply with these standards may result in the student being dismissed from Christian Homeschool Center.

I understand that our child/children are attending classes at Christian Homeschool Center at his/her own risk.

We will not hold Christian Homeschool Center, Christ the King, or any teacher, staff or volunteer affiliated with the program or the church, liable in any way.
