After reading about our program and what our role is as tutors in supporting, encouraging and helping to hold you accountable to homeschooling your child/children this year, we now ask that you would sign below if you agree with the following statements:

I have read and understand the CHC Family Handbook including the Statement of Faith, Code of Conduct, Discipline Policy and the CHC Payment Guidelines, and agree to uphold all that is written within and I understand that failure of myself as the primary homeschooling parent or my child/children to comply with all that is expected of us may result in dismissal from Christian Homeschool Center.
​​ I will not hold Christian Homeschool Center, Christ the King, or any teacher, staff or volunteer affiliated with the program or the church, liable for damages to property, injuries or any other losses.
