I have read and understand the CHC Primer including the Statement of Faith, Code of Conduct, Discipline Policy and the CHC Payments.

I understand that failure of the student and/or parent/guardian to comply with these standards may result in the student being dismissed from Christian Homeschool Center.

I understand that our child/children are attending classes at Christian Homeschool Center at his/her own risk.
​
We will not hold Christian Homeschool Center, Christ the King, or any teacher, staff or volunteer affiliated with the program or the church, liable in any way.
Signature of student or students after having read or heard CHC's Statement of Faith, Terms and Conditions, Code of Conduct, and Discipline Policy and now agrees to uphold these standards:
